Social Prescribing

Social prescribing is a healthcare approach that connects people with non-medical community resources to address social, practical, and emotional needs affecting their health and wellbeing. A “Social Prescribing Link Worker” works with the individual to identify their personal needs and goals, co-producing a plan to connect them with activities, groups, and services, such as debt advice, mental health support, or loneliness reduction through community groups. It empowers individuals to take control of their health by tapping into local community assets and is particularly beneficial for people with long-term conditions, isolation, or complex social needs.

Here at MHC we have a Social Prescriber called Abdi who is available 3 days a week, Tuesday-Thursday (excluding any bank holidays or annual leave dates). Please enquire with the Health Navigation team if you would like to meet with him.
