We have full time Care Coordinators on site, who speak with or see patients daily, and who can also help with the following areas:

Benefit support

Housing support

Immigration support

Debt support

Energy/bill support

Food poverty

Incontinence support

Form filling

Carers support/Adult care referrals

Palliative care support for pt and family

Frailty/Dementia support

Complex needs and help with navigating healthcare system

Hospital transport

Signposting to services in the community
