Visitors who are registered elsewhere are able to temporarily register at Montpelier Health Centre. They need to be staying with a patient who is already registered. They will need to complete a form prior to being seen and can be seen by either a doctor or nurse. Temporary residents can be registered for a maximum of 3 months. Please speak to a receptionist regarding this procedure and completion of a form. We will require:
- Your name
- Date of birth
- NHS number (if known)
- Permanent address
- Temporary address
- Permanent doctor’s name and address
- Period of time you will be staying in the area.
